Background and Purpose: Surgical fasciotomy has long been recognized as the definitive treatment for acute compartment syndrome. Several different methods of surgical wound closure are currently being practiced. Each method has advantages and disadvantages with regard to potential complications. The purpose of this research is to evaluate current literature regarding fasciotomy closure techniques in an attempt to find a method of closure which demonstrates superior patient outcomes. Methods: An evidence-based medicine systematic review was performed to analyze the currently available literature on fasciotomy closure techniques in the treatment of acute compartment syndrome. Studies were selected from various search engines and independently with researcher consensus vetted against inclusion and exclusion criteria. Once final articles were selected quality assessment and data extraction were performed. The data were analyzed quantitatively and qualitatively by the team of researchers, and the known data were presented. Results: After combining data from two meta-analyses and one systematic review, qualitative analysis demonstrated that dynamic dermatotraction was associated with the shortest time to closure. In contrast, negative pressure wound therapy was associated with the lowest rate of infections but the highest incidence of skin grafting. Conclusions: Although no single method can clearly be shown as all-around superior to others, dynamic dermatotraction devices lead to faster healing with low-moderate risk for overall complications.
