Background: The ABCD scoring system was developed to enhance the prediction of ischemic stroke risk after a transient ischemic attack (TIA). After reviewing reported validations of ABCD2 system, newer research conducted the utilization of the ABCD3 -I system with more specific stratification parameters in an effort to aid providers in better quality risk stratification of patients. Objective: To determine which scoring system best stratified TIA symptoms for the predictive value of stroke risk. Methods: Systematic literature review based on PubMed and Cochrane Library searches about ABCD scoring systems with the following terms: TIA, adults, stroke prevention, and screening. Our limits included: published in last 10 years, humans, full text, English, meta-analysis, and systematic review. Results: Three articles were analyzed including a systematic review, meta-analysis, and cross-sectional retrospective cohort study. Conclusion: ABCD2 scoring system has been well-established in literature;however, it lacks specificity to effectively predict post-TIA stroke risk. In order to equip non-specialist clinicians, the ABCD3 -I system should be utilized to ensure patients with TIAs are accurately stratified based on stroke risk and promote higher quality of care.
