A Systematic Review of Fenugreek Consumption for Hyperlipidemia Control in Type 2 Diabetics

Background: T2DM is a metabolic disorder that causes chronic hyperglycemia. This disease affects millions of individuals worldwide. Those with T2DM have an increased risk of developing hyperlipidemia as well. The current medications used to treat both T2DM and hyperlipidemia cause many adverse side effects. There have been studies conducted on the usefulness of herbal supplements such as fenugreek in helping lower fasting blood glucose and improving lipid profiles. Purpose: The aim of this paper is to further identify if a specific dosage and form of Fenugreek supplementation would be the most effective in improving FBG and lipid profiles. Methods: PubMed, Medline Complete, and CINAHL were searched between December 2023 - February 2024 for studies that focused on the effectiveness of fenugreek supplementation for improving FBG and lipid profiles in T2DM. Further investigation was done to ascertain dosages and forms of fenugreek used that were most effective. Results: A total of 9 studies were identified. Five of the studies reported a statistically significant reduction in fasting plasma glucose in the fenugreek intervention group compared to the placebo or control group. The most significant impact of fenugreek was observed with higher doses of the supplement. The form of fenugreek administered varied across all the studies, however fenugreek seed powder was observed in many of the studies. Four studies reported statistically significant reductions in total cholesterol, triglycerides, and low-density lipoprotein cholesterol levels, as well as an increase in high-density lipoprotein cholesterol levels among subjects with T2DM that took the fenugreek supplement as compared to placebos or control groups. v Conclusions: It can be concluded that fenugreek in seed powder form and in doses of 1 g or higher can be an effective therapy in reducing FBS, TC, TG, and LDL, while increasing HDL. For patients with uncontrolled T2DM on hypoglycemic therapy, fenugreek can be a safe and effective adjuvant therapy. However, large scale clinical trials still need to be conducted to apply to a more generalized population globally.
