Background: Lifestyle modifications, including physical activity and dietary modification, are a fundamental part of the current treatment recommendations for nonalcoholic fatty liver disease (NAFLD). Current recommendations target weight loss without an emphasis on overall dietary pattern to improve disease outcomes. Purpose: This study aims to analyze dietary approaches for the treatment of NAFLD and compare their efficacy in reducing liver fat content while improving liver enzymes and biomarkers. Methods: A literature search of PubMed, Medline Complete, and CINAHL databases was performed to collect relevant studies from the last 10 years assessing interventions in overall dietary patterns, mainly the Mediterranean diet, in patients with NAFLD. Primary outcomes of interest included liver enzymes and measures of liver fat content or liver fibrosis. However, anthropometric measurement, lipid profile, and measures of insulin resistance were also assessed as secondary outcomes. Results: Of 337 total studies, the three articles of highest quality, rated using the National Heart, Lung, and Blood Institute (NHLBI) Quality Assessment Tool, were selected for further analysis. This analysis included 4, 050 participants, with a range of 276 to 3,039 participants between studies. Adherence to Mediterranean diet (MD) was found to be a protective factor for limiting the development of liver fibrosis over time. One meta-analysis reported a statistically significant decrease in liver stiffness associated with MD interventions (P = 0.01), while another metaanalysis reported that the decrease in liver stiffness with MD intervention was not significant (P = 0.10).
